    Applicant’s Consent – By completing this application form, I agree to the collection, use and disclosure of my personal information for the following purposes, and warrant that I have supplied all information on this form freely, including my driver’s license details and passport details. I agree that any information found through any third party databases which relate to me can also be used for the purposes outlined in this application. Personal Information includes any information that identifies you or another person, that you provide, or have provided, to us or authorise us to collect at any time (including default information and updated information).  This Privacy Statement does not limit our rights and obligations under the Privacy Act 1993. 

     If you provide Personal Information about a third party to us you confirm that that the relevant person has authorised us to collect, use and disclose their Personal Information in accordance with this Privacy Statement and that you have informed them about their rights to access their Personal Information set out below. 
     It is not mandatory to provide us, or authorise us to collect, Personal Information.  However, if you do not do so, it is likely to affect our assessment of your tenancy application

     Recipient of information - The information is provided by you to the Owner/Landlord/Agent/Property Manager.

     Purposes - The purpose for the collection of my information are:

    To perform background checks, reference checks and credit checks through the Centrix, Veda and/or Dun & Bradstreet credit bureaus to assess if I am the best applicant on merit. I consent that my personal information including debts may be discussed with any personal or work referees.
    To add my information to any background, reference and credit checking database.
    To allow the Landlord to add a review to a tenancy review database that can be then viewed by other companies and/or Landlords.
    To check the Ministry of Justice fines, Tenancy Tribunal, District Court, High Court, Court of Appeal, Supreme Court, Department of Parole, and New Zealand Police databases.
    For debt collection purposes: To check the Personal Properties Securities Register. To verify my passport, citizenship, birth certificate details via the Department of Internal Affairs.
    To check if my vehicle is stolen.
    To check and update a credit bureau.
    To check whether I am politically significant.
    To update the Tenantcheck system.
    To assess your suitability for a tenancy, including assessing your and your guarantor’s creditworthiness and reference checking.
    To manage and monitor your tenancy (if your application is approved).
    To collect any amounts that you owe to us and/or enforce any Tenancy Tribunal order.

     Information Provided – We may use the services of, or provide services to any other credit reporter and/or debt collection agency, Tenancy Information New Zealand Limited (TINZ) and TINZ’s related companies (together Authorised Agencies) and any other company providing tenancy information.  We may collect Personal Information from, and disclose Personal Information to, Authorised Agencies for any of the purposes described in this Privacy Statement and we may authorise Authorised Agencies to hold, use and disclose Personal Information for the purpose of providing their (or other Authorised Agencies’) services to us and to other clients.  You agree that, subject to applicable laws, Authorised Agencies may use Personal Information to provide marketing services to their clients and Authorised Agencies and their clients may communicate with you (including by electronic means) about services and offers that they think may be of interest to you.
     Personal Information will be held by us at our business address 26 Morningside Drive, Sandringham, Auckland. You have the right to access and request correction of your Personal Information by contacting us at mark.dizon@mdgproperties.co.nz.
     The information provided on this form and any information found through the background check, reference check and credit checking process may be passed onto an agent and entered into third party databases. Subscribers to the databases will have access to my information and use that information for the purpose of credit reporting or for any other lawful purpose including debt collection. I agree that I have been advised that I have the right to see the information that you hold about me, and to have any information held corrected if it is in error. I confirm the information in this form is true, correct, accurate, complete and that I have read, understood and agree to the Privacy Statement and the Cautions under the Privacy Act 1993. I agree that if I enter a tenancy agreement then this application will form part of the tenancy agreement. I agree to pay a letting fee upon entering the premises.
